Healthier Communities and Older People

Change Theory: Creating more opportunities to enable citizens to lead healthy, active and longer lives

The Health & Social Care Partnership is responsible for the delivery of the Healthier Communities and Older People block of Oldham's Local Area Agreement. The Partnership is supported by a Healthier Communities & Older People LAA Commissioning Advisory Group, which focuses on commissioning activity to meet the targets in the LAA. The Advisory group is also responsible for the annual delivery plans and performance monitoring for LAA outcomes and indicators.
