Healthier Communities & Older People

The Healthier Communities & Older People of Oldham's Local Area Agreement has four strategic outcomes:

  1. Improve health and reduce inequalities
  2. Reduce premature mortality rates and reduce inequalities in premature mortality rates between wards / neighbourhoods with a particular focus on reducing the risk factors for heart disease, stroke and related diseases
  3. Integrated services to support independence and improved engagement of older people as active citizens
  4. Improved outcomes for diabetes.

The Oldham Partnership has commissioned activity through the Oldham Partnership Commissioning Framework which will help to deliver against the indicators within this block, and ultimately deliver the strategic outcomes listed above. Details of some of the commissioned activity is listed below:
